Have your say on the future of the NHS in Oxfordshire
The Long Term Plan sets out what the NHS wants to do better, including making it easier for people to access support closer to home and via technology, doing more to help people stay well, and providing better support for people with cancer, mental health conditions, heart and lung diseases, long-term conditions, such as diabetes and arthritis, learning disabilities and autism, and for people as they get older and experience conditions such as dementia.
Your local NHS needs to hear from you about what it should do to make care better for your community.

You can now self-refer for NHS Physiotherapy
SELF-REFERRAL FOR NHS PHYSIOTHERAPY
If you want a referral for Physiotherapy you can now refer yourself without making an appointment to see your GP. This is done via Healthshare Oxfordshire. Healthshare Oxfordshrie is your single service for all musculoskeletal problems, these are joint muscle, soft tissue injuries or back pains that are not resolving on their own.
Depending on your problem Healthshare may give you early specialist advice and help over the phone or you may be asked to go to one of their clinics for an appointment. If you need more specialist care they can arrange that within the service too.
They have ten local clinics around Oxfordshire with times and dates that should make it easy for you to visit.

If you are over 18 and are registered with The Hart Surgery or any GP in Oxfordshrie you can refer yourself via the Healthshare website

We are delighted to welcome our new GP Registrars Dr Samik Dasgupta and Dr Ram Kumar, who will be training at The Hart Surgery for six months from February 2019 to July 2019.

Patient Transport Services - New Procedure
Due to the in depth questions that we are now asked before we are able to book patient transport to and from hospital appointments we are now asking patients to book directly with the transport service. The secretaries will no longer provide this service. Please call 0300 100 0015 at least four working days before your appointment. The criteria that you have to meet to be eligible for hospital transport is determined by the Oxfordshire Clinical Commissioning Group rather than The Hart Surgery or the Ambulance Service. The criteria is subject to change but can be found here.
Those Aged 70 years
Plus anyone in their 70s who was born after 1 September 1942 and has not yet had the vaccine.
Those Aged 78 years
Plus anyone aged 79 years old who has missed out on the vaccine. These patients remain eligible for immunisation until their 80th birthday
If you are eligible for the vaccination please contact the surgery to book an appointment with a Practice Nurse.

Why we collect information about you.
As providers of Health Care we are required by law to maintain records about your health and any treatment or care you have received within any NHS organisations. We will only use the information that we hold about you for the purposes of your health care or to share information about the The Hart Surgery in a timely way.
What does this mean to you?
We already have many of your email addresses and/or mobile 'phone numbers and we will only use this information to communicate with you about matters that support your own health care or information regarding The Hart Surgery eg Newsletter, Patient Panel Group meetings.
Please check with reception we have your up to date details.
We will not disclose your information to any third party without your permission unless there are exceptional circumstances (such as risk of serious harm to yourself or others) or where the law requires information to be passed on.

Please only use our car park if you have an appointment at the surgery. We are very limited with spaces and they are reserved for our disabled, elderly, frail and acutely unwell patients. Please do not park in the allocated staff spaces as you will get blocked in and will have to disturb staff to move their cars to let you out.
Do not park at Townlands Hospital if attending an appointment at the surgery as you will receive a Parking Charge Notice and we will not be able to help you with an appeal.
Improving access to general practice: evening and weekend appointments now available
Patients who are registered at this practice can now book an appointment to see a GP or nurse on weekday evenings (after 6.30pm) or at the weekends (on Saturday and Sunday). Appointments will either take place at this practice or at another NHS setting nearby. Talk to the practice receptionist to find out more or book an appointment.
By March 2019 everyone in England will benefit from access to general practice appointments in the evenings and weekends at a time that is most convenient to them. This is part of a national drive to help improve access to general practice and get the best possible outcomes for patients. Further information is available at www.england.nhs.uk/gpaccess
PLEASE NOTE : These appointments will be aimed at routine GP care and will NOT replace the current urgent 111 service. If you require an urgent GP appointment when your surgery is closed then you still need to call 111. This service is NOT for emergencies or walk-in appointments and you will be directed to call NHS 111 in these cases.
Each surgery will be providing services at different times and on different days; however you will be able to book into an available slot at ANY of the participating surgeries. If you consent then the clinician who sees you will have access to your private medical record. Please note that if you feel an onward referral to a specialist or any further investigations may be required, then it may be better to arrange an appointment with your own doctor; however, this is not essential.
Due to the nature of these appointments we would ask that patients try and limit their queries to one problem only.
